To add a Volume Appendix, select the Add Appendix Icon 
to the right of the Appendices header.
This will open the Add / Edit Chapter Appendix window. Enter an Appendix Title, an Appendix Sub-Title (optional), select either Use Online Editor, Uploaded Document, or Upload New Document, and select the Continue Button 
.
The Appendix is now added.
