The Coversheet is a section located under each piece of equipment. The Equipment SOP Coversheet allows you to add or view information that pertains to the Procedure.

| Operator/Responsibility | Responsibilities of the Operator performing the Procedure |
| Equipment Needed | Equipment needed for performing the Procedures |
| Location | The Location of the Equipment |
| Related Documents | Any Related Documents can be listed here |
| Initial Development Date | The Initial Date this SOP was created |
| Date Revised | The most recent Revision Date (if it has been revised) |
| Revised By | The person who made the Revisions |
| Date Reviewed | The date the SOP is reviewed |
| Reviewed By | The person who conducts the Review and Certifies the Procedure |
| Annual Review | Displays Needs review/certification in Red until the procedure is Certified and is replaced by the Reviewed Date and Reviewed By information |
