The Checkboxes Group Field is used to create a group of options where multiple options can be selected.
After selecting this Risk Ranking Template, the Options List Configuration window opens. To add an option, type the option in the Add Option Field and select the Add Button 
. Add as many options as you want and select the Done Button 
.
Note: The option of having a Workorder automatically generated does not work with Forms.
