To add a Multiple Choice (Single Answer) question select the underlined Question Type.

This will open the Multiple Choice (Single Answer) page. Select the Add Question Icon 
to the right of Multiple Choice (Single Answer) header.

This will open the Edit Question: Multiple Choice (Single Answer) window. Enter the Question, an Explanation (optional), upload a photo (optional), enter the choices (up to eight), select the button next to the correct answer, and select the Save Button 
.
